ABSTRACT

ObjectiveTo investigate the clinical features of patients with hepatolenticular degeneration (HLD) aged above 35 years. MethodsA retrospective analysis was performed for the clinical data of the patients with HLD, aged above 35 years, who attended West China School of Public Health, Sichuan University, from April 2018 to April 2023, and according to their clinical symptoms, they were divided into mixed type group with 13 patients, liver type group with 12 patients, and brain type group with 5 patients. Related data were collected, including general information (sex, clinical manifestation, age at confirmed diagnosis, time from initial symptoms to confirmed diagnosis, and family history), laboratory examination (routine blood test, liver and renal function, serum copper, serum ceruloplasmin, urinary copper, and coagulation function), and radiological examination. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups; the Fisher’s exact test was used for comparison of categorical data between groups. ResultsFor the 30 patients with HLD, the male/female ratio was 3∶1, and the mean age was 46.13±5.88 years; the patients with positive Kayser-Fleischer ring of the cornea accounted for 43.33%, and the patients with liver cirrhosis accounted for 66.67%. There were significant differences between the three groups in globulin, albumin/globulin ratio, alanine aminotransferase, prothrombin time, international normalized ratio, and activated partial thromboplastin time (F=5.893, 4.513, 4.424, 5.029, 5.248, and 4.942, all P<0.05). ConclusionMost patients are male among the patients diagnosed with HLD after 35 years of age, with the main clinical types of mixed type and liver type, and such patients tend to have poor liver and coagulation functions. For unexplained liver function abnormalities and liver cirrhosis in this age group, the indicators such as serum ceruloplasmin and urinary copper should be screened as early as possible, and liver and kidney function and coagulation function should be monitored.

ABSTRACT

OBJECTIVES@#To study the etiology and clinical features of children with ascites, so as to provide a basis for the diagnosis and treatment of ascites in children.@*METHODS@#The medical data of the children with ascites, who were hospitalized from January 1, 2010 to December 31, 2019, were retrospectively reviewed.@*RESULTS@#Among the 165 children with ascites, the male/female ratio was 1.53:1, and the mean age of onset was (6±4) years. The causes of ascites included surgical acute abdomen (39 children, 23.6%), infectious diseases (39 children, 23.6%), neoplastic diseases (27 children, 16.4%), hepatogenic diseases (18 children, 10.9%), pancreatitis (10 children, 6.1%), cardiogenic diseases (8 children, 4.8%), rheumatic immune diseases (6 children, 3.6%), and nephrogenic diseases (5 children, 3.0%). According to the age of onset, there were 33 infants, 24 young children, 30 preschool children, 41 school-aged children, and 37 adolescents. Surgical acute abdomen and hepatogenic diseases were the main causes of ascites in infants (P<0.05). Neoplastic disease was the leading cause in young children (P<0.05). Infectious diseases were the most common cause in adolescents (P<0.05).@*CONCLUSIONS@#Surgical acute abdomen, infectious diseases, neoplastic diseases, and hepatogenic diseases are the common causes of ascites in children, and there are some differences in the leading cause of ascites between different age groups.

ABSTRACT

Purpose@#Resveratrol (REV), a natural compound found in red wine, exhibits antitumor activity in various cancers, including ovarian cancer (OC). However, its potential anti-tumor mechanisms in OC are not well characterized. Here, we tried to elucidate the underlying mechanisms of REV in OC cells. @*Materials and Methods@#The anti-proliferative effects of REV against OC cells were measured using CCK-8 assay. Apoptosis was measured using an Annexin V-FITC/PI apoptosis detection kit. The anti-metastasis effects of REV were evaluated by invasion assay and wound healing assay. The miRNA profiles in REV-treated cells were determined by microarray assay. @*Results@#Our results showed that REV treatment suppresses the proliferation, induces the apoptosis, and inhibits the invasion and migration of OV-90 and SKOV-3 cells. miR-34a was selected for further study due to its tumor suppressive roles in various human cancers. We found miR-34a overexpression enhanced the inhibitory effects of REV on OC cells, whereas miR-34a inhibition had the opposite effect in OC cells. In addition, we verified that BCL2, an anti-apoptotic gene, was found directly targeted by miR-34a.We also found that REV reduced the expression of Bcl-2 in OC cells. Further investigations revealed that overexpression of Bcl-2 significantly abolished the anti-tumor effects of REV on OC cells. @*Conclusion@#Overall, these results demonstrated that REV exerts anti-cancer effects on OC cells through an miR-34a/Bcl-2 axis, highlighting the therapeutic potential of REV for treatment of OC.

ABSTRACT

Objective:To investigate the role of celecoxib in inhibiting liver cancer and to explore the potential molecular mech-anisms.Methods:A total of 65 cases of patients with primary hepatocellular carcinoma(HCC) underwent surgical treatment in our hospital from February 2012 to June 2016 were recruited as study object.Another 15 cases of normal liver tissue offered by the department of hepatobiliary surgery were selected as control.Immunohistochemical staining(IHC) was used to detect the expression level of COX-2 and PD-1 in tumor and normal tissue samples.Pearson correlation analysis were used to evaluate the correlation between COX-2 and PD-1 in HCC patients.Construction of H22 hepatoma cells bearing BALB/c mice model,randomly divided them into control group,celecoxib group and PD-1 antibody group(15 mice for each group).Sacrificed all mice alive at the end of the 4th weeks after the treatment and removed the tumors then.The tumor growth curves and survival curves were drawn to observe the anti-tumor effect.IHC were used to evaluate the expression of COX-2,PD-1 as well as the number of CD8+T and foxp3 positive Treg cells in tumor tissue.Flow cytometry were used to determine the number of CD8+T and CD4+CD25+Treg cells in peripheral blood.The peripheral blood mononuclear cell(PBMC) of BALB/c mice were separated and scramble or PD-1 siRNA were transfected then,Western blot analysis were used to detected the level of COX-2,PD-1,CD8 and Foxp3 after the treatment of celecoxib.Results:IHC results showed that the expression level of COX-2 and PD-1 in tumor tissue of HCC patients were significant higher than control(P<0.001).Pearson correlation analysis showed COX-2 were both positive correlated with PD-1 in the tumor tissue of HCC patients(R2=0.673,P<0.001).The tumor growth curves and survival curves in celecoxib or PD-1 antibody groups were significant better than in control group (P<0.05).There were no significant difference of tumor growth curves and survival curves between celecoxib group and PD-1 antibody group(P>0.05).IHC and flow cytometry analysis showed that celecoxib treatment significant decreased the expression level of PD-1(P<0.05).Both celecoxib and PD-1 antibody treatment significant increased the number of CD8+T cells while decreased the number of Treg cells in the tissue or the peripheral blood(P<0.05).Western blot analysis showed that celecoxib significant decreased the level of COX-2,PD-1,CD8 and Foxp3 in PBMCs,while it did not affect the level of CD8 and Foxp3 in PD-1 siRNA transfected PBMCs.Conclusion:The expression level of COX-2 and PD-1 were increased in the tumor tissue of HCC patients.Celecoxib may inhibit liver cancer by regulating PD-1 mediated tumor immune via inhibiting COX-2.

ABSTRACT

Through the clinical research information sharing system,this study was aimed to explore the law of Professor Shen Zhongyuan's treatment of hepatitis B liver cirrhosis,and to inherit academic thoughts of senior famous traditional Chinese medicine (TCM) doctors.The structured electronic medical record system was used.The information of Professor Shen Zhongyuan expert outpatient with hepatitis B cirrhosis was collected from September 2012 to September 2015.And then,the medical data were extracted and analyzed to find out the medication regularity of hepatitis B cirrhosis and the distribution rule of the taste tropism of herbal medicine in the treatment.The results showed that 388 cases of hepatitis B cirrhosis patients with a total of 127 kinds of herbal medicine.The cumulative using frequency of herbal medicine was 5595.The most commonly used 10 types of Chinese medicine were turmeric,citrus aurantium,cuttlefish bone,atractylodes,rhizoma coptidis,salvia miltiorrhiza,lobelia,artemisia capillaris,fructus trichosanthis,and chicken's gizzard-membrane.The taste of mostly used Chinese medicine is bitter and sweet,followed by acrid.The nature of Chinese medicine is cold,slightly cold,warm and even.The meridian tropism of Chinese medicine is mainly to the liver and spleen meridian.It was concluded that Professor Shen in treatment of hepatitis B cirrhosis drug laws was often to use heat-clearing and toxin-relieving,qi-reinforcing and spleen-invigorating medicine,liver-soothing and qi-regulating medicine,blood-activating and stasis-removing medicine.The nature of herbal medicine used is mostly cold,slightly cold,warm and even.It is based on the TCM syndrome differentiation and treatment thought of “regulating yin-yang to maintain the balance.” It showed the important principle of “toxin-removing,stasis-removing,liver-regulating and spleen-strengthening” by professor Shen.

BACKGROUND: Steroid receptor coactivator-3 (SRC-3) is a member of the steroid receptor coactivator family,and it can promote proliferation and differentiation in human osteoblasts by modulating estrogen receptor activity. Previous studies have reported that allelic variation at the SRC-3 locus is significantly positively correlated with the lumbar bone mineral density in Caucasian men, but the relationship between SRC-3 and bone metabolism in postmenopausal women remains unknown.OBJECTIVE: To investigate the level of serum SRC-3 in postmenopausal women and its association with bone mineral density and bone turnover markers.METHODS: Fifty-five women with postmenopausal osteoporosis and 35 healthy postmenopausal women were recruited, and their serum levels of SRC-3, 25-hydroxyvitamin D, osteocalcin and procollagen I N-terminal peptide, beta C-terminal telopeptide of type I collagen were detected. The bone mineral density of L1-4 vertebrae and femoral neck were measured by dual-energy X-ray absorptiometry.RESULTS AND CONCLUSION: Serum SRC-3 level in postmenopausal women with osteoporosis was remarkably lower than that in healthy individuals. In addition, the serum SRC-3 level was positively correlated with lumbar bone mineral density, and negatively correlated with procollagen I N-terminal peptide and osteocalcin in postmenopausal women with osteoporosis. These results indicate that low serum SRC-3 level may be involved in the pathogenesis of postmenopausal osteoporosis and play a pivotal role in bone turnover.

ABSTRACT

AIM: To investigate the effects of vitrectomy combined with cataract surgery on the corneal endothelial cells in diabetic retinopathy.METHODS: A retrospective study was designed.160 patients(160 eyes) with diabetic retinopathy from Jan 2015 to Feb 2017 were divided into two groups according to cataract.74 patients(74 eyes) were operated on vitrectomy,and 86 patients(86 eyes) on vitrectomy combined with phacoemulsification cataract surgery and capsular bag implantation of foldable intraocular lens.To record the change of corneal endothelial cells density,average cellular area,coefficient of variation and percentage of hexagonal endothelial cell before and after treatment with Topcon corneal specular microscope.RESULTS: Before and after surgery,the results of corneal endothelial cells density,average cellular area,coefficient of variation and percentage of hexagonal endothelial cell in simple vitrectomy group were no significant difference(P>0.05);After treatment corneal endothelial cells density and percentage of hexagonal endothelial cell were changed with statistical difference as the same as average cellular area and coefficient of variation(P<0.05);There were significantly differences in corneal endothelial cells between two groups(P<0.05).CONCLUSION: It has certain influence on the corned endothelial cells when using vitrectomy combined with cataract surgery in diabetic retinopathy.For patients with indications,it should be paid attention to protecting the corneal endothelial cells.

ABSTRACT

Brazilein is an active small molecular compound extracted from Caesalpinia sappan L. with favorable pharmacological properties on immune system, cardiovascular system, and nervous system. C. sappan has been used as a traditional medicine in China for hundreds of years for various diseases. However, the general reproductive toxicity of brazilein is still unknown. The purpose of the present study was to thoroughly evaluate the general reproductive toxicity of brazilein in ICR mice to support the future drug development and modernization of this potent traditional Chinese medicine. The results showed that, although no apparent toxicity on the reproducibility of the male was observed, brazilein might cause considerable risks to the fetuses and females as indicated by the ratios of dead fetuses and reabsorptions. In conclusion, our results from the present study provided some useful insights about the safety profile of brazilein, suggesting that brazilein should be used with caution in pregnant women.

ABSTRACT

Aim To investigate the effect of TRPV4 on hepatic fibrosis of rats . Methods Liver fibrosis model of rats was induced by 50% CCl4 twice a week for 12 weeks. HE and Masson staining were used to evaluate the degree of hepatic fibrosis, and the levels of α-smooth muscle actin(α-SMA) and TRPV4 were detec-ted in fibrotic liver tissue by Western blot. HSC-T6 cells were activated by transforming growth factor β1 (TGF-β1),and the protein levels of α-SMA, TRPV4 were detected by Western blot. After using Ru and transfected with TRPV4-siRNA, HSC-T6 was stimula-ted with TGF-β1, the levels of α-SMA, TRPV4 and phosphorylation level of Akt were determined by West-ern blot. Results TRPV4 was highly expressed in model liver tissues and in activated HSC-T6 induced by TGF-β1 . The levels of α-SMA and phosphorylation of Akt decreased in TGF-β1-induced HSC, used with Ru or transfected with TRPV4-siRNA. Conclusions The expression of TRPV4 increases in fibrotic livers and ac-tivated hepatic stellate cells. Knockdown of TRPV4 can suppress the activation of hepatic stellate cells in-duced by TGF-β1 , and decrease the phosphorylation levels of Akt.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the application of the expanding forming under the plate through cervical spatium intermusculare approach to treat multi-segmental myelopathic cervical spondylosis.</p><p><b>METHODS</b>From July 2005 to June 2013, 25 patients with multi-segmental myelopathic cervical spondylosis were treated by the expanding forming under the plate through cervical spatium intermusculare approach including 16 males and 9 females with an average age of 56.5 years old ranging from 35 to 78 years old. Among them, 10 cases were onset without causes slowly, 7 cases were onset without causes suddenly, 8 cases were onset after mild trauma or tired. JOA scoring, incidence of postoperative axial symptoms and imaging studies were used to evaluate the effect.</p><p><b>RESULTS</b>Twenty-five cases were followed up for 6 months to 7 years and 6 months with an average of 2 years and 9 months. There were no infection, cerebrospinal fluid leakage after the operation, and complications such as nerve damage were occurred. The operation time was 120 to 150 min, the bleeding was 300 to 500 ml. Imaging examination showed vertebral canal sagittal diameter increased, the vertebral canal increased significantly in the cross sectional area of the spinal cord, cervical curvature was straighten in 4 cases (2 cases of them became normal sequence). There were no more cases of cervical protruding and segmental instability increased. Postoperative walking ability enhanced, the finger activity of majority of patients improved on flexibility, grip strength, and accuracy of using chopsticks improved, numbness and chest waist band feeling had different degree of reduce, preoperative urine impairment were improved to varying degrees. Preoperative JOA scores were 3 to 13 points with an average of (8.86 ± 4.25) points; Postoperative 12 months' JOA scores were 7 to 17 points with an average of (13.76 ± 3.56) points, period was 60.19% in average, JOA score had statistically difference between before and after operation (P < 0.05). The result were excellent in 14 cases, good in 6 cases, and fair in 4 cases and poor in 1 case.</p><p><b>CONCLUSION</b>Application of the expanding forming under the plate through cervical spatium intermusculare approach to treat multi-segmental myelopathic cervical spondylosis, through muscular clearance, can retain complete cervical back muscle, maintain stability of cervical spine, and vertebral canal can get effective decompression at the same time.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the surgical method and its effects of adjacent spinal segment degeneration after anterior cervical fusion.</p><p><b>METHODS</b>From March 2000 to March 2011, 27 patients with spinal segment degeneration who had accepted the operation of anterior cervical fusion were treated with surgical treatment. There were 16 males and 11 females with an average age of 55.3 years (ranged from 48 to 72 years). JOA scores and image examination were used to evaluate the clinical effects.</p><p><b>RESULTS</b>All patients were followed up for 1.8 to 7.2 years with an average of 3.6 years. All pathological segments obtained fully decompression with good spinal bombe, no internal fixation loosening and cervical spine instability were found. Nerve root pain had disappeared and nerve function had significantly improved. Before operation, 3 days after operation and at last follow-up, JOA scores were 9.15 +/- 3.46, 13.96 +/- 2.79 and 13.52 +/- 2.91, respectively, and there was significant difference between preoperation and postoperation (P < 0.05). Intervertebral height and physiological curvature improved obviously than preoperation (P < 0.05).</p><p><b>CONCLUSION</b>In order to relief spinal compression and rebuild spinal stability, a surgical treatment will be recommend to adjacent spinal segment degeneration combining spinal nerve symptoms and physical sign as soon as possible. Selecting an appropriate surgery to treat different compressed segment would receive satisfactory results.</p>

Objective To evaluate the MUC1 promoter's role in driving gene expression in pancreatic cancer and its therapeutic significance.Methods Two plasmids were made.The plasmid pEGFP-MUC1N1 contained MUC1 promoter fragment connected to the pEGFP-N1 vector with the EGFG reporter gene.The pShuttle-MUC1-EGFP plasmid contained MUC1 promoter fragment and EGFP reporter gene connected to pShuttle plasmid.Lipofectamine 2000 was used to transfect the two plasmids into cells of MUC1-positive human pancreatic cell line Panc-1 and MUC1-negative human cervical carcinoma Hela.Fluorescence microscopy and flow cytometry compared the specificity and activity of the MUC1 promoter and CMV promoter.Results Reporter gene EGFP-positive cells 48 hours after transfection with pEGFP-MUC1-N1 and pShuttleMUC1-EGFP plasmid were 69.6% and 63.6% respectively,in Panc-1 cells,and 4.2% and 3.7% respectively,in Hela cells.Conclusions MUC1 promoter can drive reporter gene activity in MUC1-positive tumor cells targeting functional expression.There is potentially a use of targeted therapy in pancreatic cancer at the genetic level.

Objective To investigate the effects of KAP-1 in promoting the epithelial-mesenchymal transition (EMT) of pancreatic cancer cell line Capan-2.Methods The lentiviral vector of LV-plenti-GFP-KAP-1 was constructed.Capan-2 cells were divided into the experimental group (cells transfected by lentiviral vector of LVplenti-GFP-KAP-1),negative control group (cells transfected by empty vector) and blank control group 1 (cells cultured in 1640 medium plus 10% fetal calf serum).Capan-2 cells in the experimental group were subdivided into the miR-100-5p inhibitor transfection group (cells transfected with miR-100-5p inhibitor),empty vector control group (cells transfected with microRNAs inhibitor),blank control group 2 (cells cultured in 1640 medium plus 10% fetal calf serum).The lentivirus was identified by double endonuclease restriction and sequencing,and the virus titer was detected.The morphological changes of the cells were observed after transfecting lentiviral vector of LV-plenti-GFP-KAP-1 to the Capan-2 cells.The expressions of KAP-1,genes of EMT proteins and mRNA of miR-100-5p were detected by real-time quantitative polymerase chain reaction.The protein expressions of KAP-1,EMT proteins in all the groups were detected by Western blot.The measurement data were presented by mean ± standard deviation,and were analyzed using the analysis of variance.Results The lentiviral vector of LV-plentiGFP-KAP-1 was successfully constructed,and the virus titer was 2 × 108 TU/ml.Compared with the control group,the mesenchymal transition of the Capan-2 cells was detected in the experimental group after transfecting the Capan-2 cells with lentiviral vector of LV-plenti-GFP-KAP-1 for 48 hours.The relative mRNA expressions of KAP-1,N-cadherin,vimentin,E-cadherin,miR-100-5p were 1.77 ± 0.83,2.62 ± 0.71,2.50 ± 0.21,7.20 ± 1.17 and 1.81 ±0.40 in the experimental group,5.03 ±0.29,5.07 ±1.53,3.83 ±0.57,7.83 ±0.78,7.01 ± 0.96 in the negative control group,5.13 ± 1.14,5.81 ± 1.49,4.92 ± 0.90,3.07 ± 0.36,6.87 ± 0.35 in the blank control group 1,with significant difference among the 3 groups (F =5.99,7.62,7.88,6.62,4.64,P <0.05).The relative mRNA expression of KAP-1 in the miR-100-5p inhibitor transfection group,empty vector control group,blank control group 2 were 1.56 ± 0.42,4.89 ± 0.61,5.20 ± 0.38,with significant difference among the 3 groups (F =5.14,P < 0.05).The relative mRNA expressions of vimentin in the miR-100-5p inhibitor transfection group,empty vector control group,blank control group 2 were 3.10 ± 1.37,3.44 ± 0.94,3.08 ±1.16,with no significant difference among the 3 groups (F =0.49,P > 0.05).The results of western blot showed that the relative protein expressions of KAP-1,N-cadherin,vimentin,E-cadherin were 2.77 ± 1.99,1.31 ±0.38,4.25 ± 0.63,0.62 ± 0.06 in the experimental group,0.83 ± 0.46,0.41 ± 0.37,1.03 ± 0.33,1.17 ± 0.45 in the negative control group,0.71 ± 0.26,0.08 ± 0.04,1.37 ± 0.92,3.04 ± 0.65 in the blank control group 1,with significant difference among the 3 groups (F =5.54,4.68,3.19,8.18,P < 0.05).The relative protein expression of KAP-1 in the miR-100-5p inhibitor transfection group,empty vector control group,blank control group 2 were 2.27 ±0.71,0.56 ±0.43,0.61 ±0.39,with significant difference among the 3 groups (F =4.81,P <0.05).The relative protein expressions of vimentin in the miR-100-5p inhibitor transfection group,empty vector control group,blank control group 2 were 3.19 ± 0.55,3.93 ± 0.06,3.61 ± 0.73,with no significant difference among the 3 groups (F =0.04,P > 0.05).Conclusion KAP-1 promotes the EMT of Capan-2 cells by specifically down-regulating the miR-100-5p expression.

ABSTRACT

Objective To investigate whether KAP-1 could induce CSC-self renewal in pancreatic cancer cell lines.Methods KAP-1 expression was examined in 14 cases of pancreatic cancer with immunohistochemistry.KAP-1 shRNA amplified by PCR was inserted into pGC-LV in vector,and then identified by restriction endonuclease digestion and nucleotide sequencing.The lentiviral vector pGC-shRNA-KAP-1 was co-transfected with pHelper 1.0 and pHelper 2.0 packaging plasmids into HEK 293T cells,and the lentivirus was collected and virus titer was measured.The expressions of KAP-1 and vimentin were detected by Western blot and RT-qPCR when human pancreatic cancer cell line Panc-1 was infected by the lentivirus.Sphere forming assay was conducted to assess the capacity of CSC or CSC-like cell self-renewal in this study.Results The KAP-1 expression level in cancerous tissues on immunohistochemistry was significantly higher than in the corresponding normal tissues (P=0.002).After infected by lentivirus,the expressions of KAP-1 and vimentin were knocked down,which could be detected by Wesren blot and RT-qPCR.Compared to Panc-1-GFP (NC) cells,the outcomes suggested that knocking down the expression of KAP-1 could decrease the formation of pancreatospheres,which further suggested the capacity of CSC-self-renewal in primary and secondary pancreatospheres of Panc-1-shRNA-KAP-1 and NC cells.Conclusions KAP-1 expression in pancreatic cancer tissues has been identified.The lentiviral vector for shRNAs targeting KAP-1 was constructed successfully.The formation of pancreatospheres decreased by knocking down the KAP 1 gene.KAP-1 is involved in the regulation of CSC phenotype.The mechanism is probably related to the upregulated expression of the EMT marker vimentin.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the therapeutic effectes of vacuum sealing drainage (VSD) technique and reattachment of avulsed skin in treating avulsion injuries of limbs.</p><p><b>METHODS</b>A retrospective analysis was done on 25 patients suffering from avulsion injuries of limbs, who were treated with vacuum sealing drainage and reattachment of the avulsed skin. Among the patients, 19 patients were male and 6 patients were female, ranging in age from 21 years to 57 years, with an average of 41 years. After debridement, the patients got reattachment of skin graft with subdermal vascular network or split-thickness skin graft, and the wound surface was covered with polyvinyl sponge. VSD was removed after 7 to 14 days. The color, survival rate and sensory function of the skin were observed after operation.</p><p><b>RESULTS</b>All the wounds infection was controlled with VSD. The skin survival rate was more than 96% in 16 cases, more than 90% in 7 cases, and skin edge of 2 cases had little necrosis but rehabilitated after dress-changing. The reattached skins had almost normal skin color, soft and abrasion resistant, satisfactory sensory function, and no compression ulcer was observed after 4 weeks.</p><p><b>CONCLUSION</b>VSD combined with graft with subdermal vascular network or split-thickness skin graft helps to promote wound drainage, reattachment of skin, and decrease infection. It is an effective method in treating avulsion injuries of limbs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the therapeutic effects of percutaneous vertebroplasty with Genex combined with pedical screw fixation in the treatment of thoracolumbar burst fractures.</p><p><b>METHODS</b>From February 2007 to October 2009, 38 patients with thoracolumbar burst fractures were treated with percutaneous vertebroplasty with Genex and pedical screw fixation. There were 25 males and 13 females, with a mean age of 42 years old (ranged, 30 to 68 years). The cause of injuries included road accident of 9 cases,crush injury of 7 cases,crash of 21 cases and others of 1 case. The fractures were classified according to Denis classification: 6 cases of type A, 22 cases of type B, 7 cases of type C and 3 cases of type D. Functional assessment of nerves was assessed according to ASIA criteria: 4 cases of type A,7 cases of type B, 10 cases of type C, 6 cases of type D and 11 cases of type E. Therapeutic effects were assessed by imaging and ASIA standard.</p><p><b>RESULTS</b>All the patients were followed up, and the duration ranged from 6 months to 34 months,with a mean period of 16.5 months. The results were observed after 1 week by X-ray inspection that vertebrate column array of all patients were restored to normal, anterior height of centrum restored to (98.50 +/- 2.17)%, posterior height of centrum for (98.87 +/- 1.82)%; and Cobb angle of injured vertebra decreased to average of (1.63 +/- 2.15) degree; the stenosis rate restored to (1.45 +/- 3.47)%. By X-ray inspection, it was observed that the vertebral height kept good at 6 months after operation, and the bone density almostly normal. By CT scanning, it was observed that bone defects of vertebrae disappeared, and restored to normal bony tissue. The nerve function restored 1 to 2 grades 6 months after operation.</p><p><b>CONCLUSION</b>Artificial bone of Genex applied in vertebral body-plasty has good biocompatibility, strong bone inducibility, no complications, little loss of vertebrae height and Cobb angle, and satisfactory results in the near future.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the therapeutic effectiveness of vacuum sealing drainage (VSD) technique and mesh grafting in treating defects of skin and soft tissues in foot.</p><p><b>METHODS</b>A retrospective analysis was done on 17 cases (11 male and 6 female) suffering from defects of skin and soft tissues in foot, which were treated by vacuum sealing drainage and mesh grafting. The age of patients was from 18 to 67 years with an average of 43 years. The wound surface was filled with polyvinyl alcohol gelatin sponge after debridement and continuous negative pressure drainage was taken for 24 h. After 7 days, granulation tissue growing mesh grafting was performed and to observe the skin colour, survival rate and feet function.</p><p><b>RESULTS</b>All the infection of wounds was controlled with VSD for 1 to 3 times. Skin survival rate of 14 cases more than 98%, 2 cases more than 95%; skin edge of 1 case had little necrosis, but foot function obtained rehabilitation after dress-changing.</p><p><b>CONCLUSION</b>Vacuum sealing drainage (VSD) technique and mesh grafting is effective methods for the treatment of defects of skin and soft tissues in foot and is worthy generalization and application.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical effect of Hook-screw combination treatment for the unstable Hangman's fracture.</p><p><b>METHODS</b>From March 2005 to May 2008, 17 patients with unstable Hangman's fracture, of whom there were 15 males and 2 females aged from 28 to 75 years old with an average age of 46 years, were treated by Hook-screw combination. Of injury reasons, there were 8 patients caused by falling injury, 5 patients caused by traffic accident injury, 3 patients by tumble and 1 patient by crash injury of heavy object. Type of bone fracture: there were 3 patients with type II, 5 patients with type I a and 9 patients with type Ill. The level of nerve injury according to ASIA criteria, there were 3 patients with grade C, 5 patients with grade D and 9 patienst with grade E.</p><p><b>RESULTS</b>All patients were followed up for 3 to 22 months averaged 8 months. All patients were recover completely without upper cervical spine pain and C2, C3 loss of reduction. No patients' nerve function was injured in the patients with normal function before operation. The patients with injured nerve function before operation improved 1 to 2 grade.</p><p><b>CONCLUSION</b>The method of transarticular Hook-screw combination treatment for the unstable Hangman's fracture can obtain effective anatomical reduction and biological stability.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the operative methods for fracture and dislocation of lower cervical spine with articular process interlocking.</p><p><b>METHODS</b>From Feb. 2002 to Feb. 2008, 57 patients with fracture and dislocation of the lower cervical spine with articular process interlocking were treated by operation involving interior, posterior and combining anterior and posterior approach, of whom there were 45 males and 12 females aged from 18 to 73 years old with average of 39.5 years. There were 35 patients caused by traffic accident, 17 patients by falling injury and 5 patienst by crash injury of heavy object. Injured cervical segment: there were 4 patienst with C3-C4, 13 patients with C4-C5, 32 patientst with C5-C6 and 8 patients with C6-C7. Fracture types of cervical dislocation: there were 32 patients with unilateral articular interlocking and 25 patients with bilateral articular interlocking. According to ASIA criteria the level of nerve injury was grade A in 9 patients,grade B in 17 patients, grade C in 19 patients and grade D in 12 patients. OPERATIVE METHODS: 14 patients were selected by anterior operation, 11 by posterior operation,and 32 by combining anterior and posterior operation.</p><p><b>RESULTS</b>All patients were followed up for 3 to 26 months averaged 11 months. According to ASIA criteria, all patients improved 1 to 2 grade. Patients with cervical fracture and dislocation recovered completely with normal cervical sequence and cervical curvature. There was no patients with cervical unstability. Bony fusion were found in the patients with anterior interbody fusion at 12 weeks after operation.</p><p><b>CONCLUSION</b>The anterior operation was selected when one segment fractures on fresh and dislocation, or the cervical fracture and dislocation were reduction with traction, or the angulation deformity was severe. The posterior operation was selected when the cervical vertebral plate of articular facet fractures and the compression were from posterior, or multi-segment of cervical injured. The combining anterior and posterior operation was selected when the compression of spinal cord was from both side.</p>
